All Semiahmoo Team Officials - coaches, managers, trainers, female practice volunteers, etc. must be registered with the club (see Team Contact Form below) AND have completed the BCSA RISK MANAGEMENT FORM(S) prior to helping with any club teams.
1. BCSA Risk Management Forms
District ID Cards will not be processed for team officials who have not completed Risk Management Forms.

Must be completed by ALL team officials
Risk Management Form B - Click Here to Complete Form B Online*
Must be completed by all Team Officials who will be handling team finances.
*The Risk Management forms are completed online with South District Girls Soccer Association.
2. Team Officials Contact Information Form
Complete the club form to register both your Team Name and your Team Officials.
Please enter information for all your team officials at one time.
If you have more than 4 team officials to register, submit the first four and then complete the form again for any remaining officials.
- NOTES:
A minimum of two team officials are required per team.
One official must be female.
